Cornerstone VNA is an independent, nonprofit home health and hospice organization founded in 1913, serving over 40 communities in New Hampshire and Maine. Like most home health agencies, Cornerstone VNA relied on traditional manual wound assessment tools and documentation, which created challenges in standardization and visibility of wound care in the home. While sending wound experts and more experienced nurses into the field helped to provide better care for complex wound patients, this model was ultimately unscalable due to the increased cost and scarcity of these clinicians.

The Swift Medical platform allowed frontline clinicians to easily capture high precision, clinically-calibrated wound images on their smartphone devices and seamlessly capture documentation required for standardized wound tracking and virtual consultations.

A benefits evaluation study was completed after the adoption of Swift Skin and Wound Care solution at Cornerstone VNA. The study compared clinical, operational and economic outcomes for the same 5-month period (April-August pre-and post-adoption of the solution).
