Nursing Innovation Leader of the Year Award

The Award

This award will offer $5,000 in cash and $37,000 in prizes to a nurse leader who has demonstrated applying new ideas, methods and/or technology to advance practice, education, clinical outcomes and/or patient experience.

Requirements

Applicants for Nurse Innovator of the Year Award must be a nursing leader (Executive, Director, Manager or Clinical Leader) currently employed at a healthcare organization in the United States. Healthcare organizations can be a Hospital, Long Term Acute Care Hospital (LTAC), Skilled Nursing Facility, Wound Care Clinic, Hospice or Home Health. Further terms and conditions apply, see here.

Application is submitted online and nominations must include:

  • Description of the issue, background and important of the issue(s) that was addressed with the innovation
  • What was the motivation and inspiration to drive this change?
  • Description of the innovation, how it was implemented and by whom (if a team was involved)
  • The resulting impact on practice, education, clinical outcomes and/or the patient experience
  • The evaluation methods and thoughts on how the innovation can be scaled and spread to others

Terms & Conditions

Overview

The Nursing Innovator of the Year Award (“Award”) takes place between February 20th, 2020 and Mat 12, 2020. The Award starts with a nomination process in which individuals will nominate an innovation leader for consideration as part of the program. A group of independent judges will evaluate each entry and provide a score based upon the entry material and the judging criteria.

Eligibility

Nominees must be employed by a non-profit, for-profit or government agency in the United States healthcare industry including: Hospitals, Long Term Acute Care Hospitals (LTAC), Skilled Nursing Facilities, Wound Care Clinics, Hospice or Home Health. All nominees must be aged 18 years or over.

Nomination Process

Individuals, a corporation or governmental may nominate someone for the awards program via the online submission starting February 20th, 2020 and complete the process prior to 5PM Pacific Time (PT) on March 31, 2020 for consideration. Upon submission, each individual will be entered into the Awards. The form shall include the nominees name, organization, title, email address and information on their work for recognition based upon the Award criteria.

Each nominee, (or if not self-nominated, upon accepting the nomination), shall hereby accept the terms and conditions of the award. By entering the Award, entrants and nominees agree to consent to Swift Medical using the personal information submitted for the purposes of evaluation and judging as part of the competition.

Failure to claim or accept the nomination, or meet the eligibility criteria may result in disqualification or forfeiture of the prize.

Judging, Finalists and Winners

All submissions will be scored by the judges on a ranking system for each of the categories and all entries will be tabulated and aggregated. The 10 highest scores will be recognized as finalists and be announced between May 1st, 2020 and May 11th, 2020. In the event of a tied score at any point of the competition, an unbiased employee from Swift Medical will act as an additional judge to evaluate and determine the winner. The final winner will be announced on May 12th, 2020 on the International Day of Nurses.

Prizes

$5000 Cash will be paid to the winner and can be used any way they wish.

Cancelation or Termination of Awards

The sponsors reserve the right to cancel, suspend and/or modify the Award, or any part of it, if any fraud, technical failures, virus, human error, tampering or any other factor impairs the integrity or proper functioning of the Award, as determined by the sponsors in their sole discretion. In such an event, the sponsors reserve the right to award the prize from among the eligible entries received up to the time of the impairment. The sponsors reserve the right, in their sole discretion, to disqualify any individual it finds to be tampering with the entry process or operation of the award, or to be acting in violation of the Award terms and conditions of this or any other promotion in a disruptive manner.

Any attempt by any person to deliberately undermine the legitimate operation of the Award may be in violation of criminal and civil law, and, should such an attempt be made, the sponsors reserve the right to seek damages from any such person to the fullest extent permitted by law. The sponsors’ failure to enforce any term of these Official Rules shall not constitute a waiver of that provision.

Limitation of Liability

The sponsors are not responsible for interrupted or unavailable network servers or other connections; for errors in communication protocols; failed telephone, computer or electronic transmissions; for jumbled, scrambled or misdirected entries or transmissions; for phone, electrical, network, technology or computer hardware, software or program malfunctions, failures or difficulties; or any other errors, omissions, interruptions or deletions of any kind, whether human, mechanical or electronic; or for non-receipt of any entry regardless of the cause of it not being properly received.

The sponsors are not responsible for unintelligible, late, lost or stolen entries; for incorrect or inaccurate entry information, whether caused by internet users or by any of the equipment or programming associated with or utilized in the Award; or for any technical, printing or human errors which way occur in the receipt, non-receipt or processing of any entries in the Award.

THE SPONSORS MAKE NO WARRANTIES OF ANY KIND, EITHER EXPRESS OR IMPLIED, INCLUDING, BUT NOT LIMITED TO, IMPLIED WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE. Privacy Policy

Any personally identifiable information collected during an applicant’s participation in the Awards will be collected by Swift Medical and used by Swift Medical in accordance with Swift Medical’s Privacy Policy as such as stated at https://swiftmedical.com/privacy/. Swift Medical will share the nominator’s name with the nominee but not with the judges not with the general public. Beyond the usage described in these terms and conditions, the information provided will be collected, stored and used in accordance with Swift Medical’s privacy policy.

Publicity Rights

By submitting nomination, nominees (or persons entering on behalf of nominee) shall allow the sponsors and/or the sponsor’s designees the perpetual right to use his/her entry name, biographical information, photos, videos and/or likeness, and statements for promotion, trade, commercial, advertising and publicity purposes, at any time or times, in all media now known or hereafter discovered, worldwide, including but not limited to on the World Wide Web and Internet, without notice, review, approval and without additional compensation except where prohibited by law. A signed affidavit of eligibility, publicity release and release of liability may be required.

Governing Law

Issues concerning the construction, validity, interpretation and enforceability of these Official Rules shall be governed by the laws of Illinois.

Release

By participating, each nominee releases and agrees to indemnify and hold harmless the Award Sponsors from and against any and all costs, claims, damages, or any other injury, whether due to negligence or otherwise, to person(s) or property (including, without limitation, death or violation of any personal rights, such as violation of right of publicity/privacy, libel, or slander), due in whole or in part, directly or indirectly, to participation in the Awards.

General

The sponsors reserve the sole right to amend or alter the terms and conditions of the Award at any time. By entering the Award, nominees agree to abide and be bound by these Official Rules and the decisions of Swift Medical, which shall be final in all matters pertaining to this Award program. All entries become the property of the sponsors and will not be returned. Void outside of the fifty (50) United States/District of Columbia and wherever prohibited or restricted by law. All federal, state and local laws and regulations apply.
