Introduction: Many home health agencies leverage a digital wound management solution to support wound care practice improvement, visit efficiency and enhance clinical capacity. Leveraging Swift’s large, clinically calibrated wound database, this quality improvement study aimed to understand the efficiency of completing a wound care evaluation among a group of home health agencies that implemented the solution from 2021-2023.

Objective and methods: This study examined 569,494 de-identified wound care evaluations from 29 US home health agencies who had adopted Swift Skin & Wound. The results were compared across three years from 2021 to 2023.

Results:

  • 55% improvement in the time to complete a wound evaluation.
  • 36% improvement in the timeliness to complete a wound evaluation in 60 minutes.
  • 15% improvement in the timeliness to complete a wound evaluation in 24 hours.
